Mrs. M., a 72 year old female was admitted to our facility with a diagnosis of Encephalopathy on October 10, 2016. She also has a past medical history of urinary tract infection, Coronary Artery Disease, DM II, and hypertension. Upon admittance Mrs. M. demonstrated balance deficits, decreased activity tolerance, and complaints of left shoulder pain. She also had difficulty with short term recall and required a modified diet secondary to dysphasia.
Mrs. M. received Physical Therapy five times a week for eight weeks. Upon discharge from Cumberland Nursing and Rehabilitation, Mrs. M. was ambulating unlimited distances without the use of an assistive device. She also improved on the Functional Reach Test from four inches to twelve inches and she was able to maintain single limb stance for ten seconds which all indicated a low risk for falls.
Mrs. M. also received occupational therapy five times a week for eight weeks. Following her occupational therapy treatment, Mrs. M. was able to tolerate functional tasks for ten minutes. She also achieved modified independence with tasks such as toileting and lower body dressing and improved safety during tasks. Pain in left shoulder improved from 4/10 on pain scale to 2/10 allowing for functional tasks to be performed.
After receiving speech therapy five times a week for four weeks. Mrs. M. is able to recall life information with 90% accuracy and was safely tolerating a regular diet with thin liquids.
It was truly a pleasure to serve Mrs. M.. She was motivated to reach her prior level of functional independence. She has been an inspiration to other patients in this facility and will be greatly missed. However, the entire staff at Cumberland Nursing and Rehabilitation was thrilled for her to transition to an assisted living facility on December 1, 2016.
