Mrs. Thomas, a 78 year old female, was diagnosed with a right femur fracture and right ulna fracture with an external fixator in place. The patient also had a past medical history of CABG, arthritis, glaucoma, and depression. Upon admission to our facility, Mrs. Thomas was transferring with total dependence and was unable to ambulate secondary to non-weight bearing status of right upper and lower extremities. Mrs. Thomas required maximal assist with bathing, toileting, upper body dressing, and total dependence with lower body dressing. Mrs. Thomas demonstrated noted difficulties in the areas of short term memory, problem solving and sequencing.
Mrs. Thomas received physical therapy five times a week for fourteen weeks. Following her physical therapy Mrs. Thomas was ambulating 125 feet with rolling walker and contact guard assistance. She was able to transfer supine to sit with contact guard assist and stand pivot transfers with minimal assistance. Her physical therapy also focused on increasing right knee flexion/ extension for improved functional ability. Right knee range of motion improved from -15-43 to -10-82 degrees.
Mrs.Thomas also received occupation therapy five times a week for fourteen weeks. Upon discharge from Cumberland Nursing and Rehabilitation, Mrs. Thomas was able to safely and efficiently perform upper body dressing with supervised assist and grooming/hygiene tasks were performed with set-up assistance. Therapy sessions also focused on improving functional use of the right wrist and elbow with decreased pain. Her pain level decreased 10/10 on pain scale to 4/10 at time of discharge.
After receiving speech therapy fixe times a week for twelve weeks, Mrs. Thomas demonstrated improvement with short term memory from 70% to 90% and her functional problem solving skills improved from mild to minimal/close supervision.
It has truly been a pleasure to serve Mrs. Thomas. She was motivated to reach her highest potential in order to return home with family. She has been an inspiration to others and will be greatly missed, however, the entire staff at Cumberland Nursing and Rehabilitation appreciates the opportunity to have worked with Mrs. Thomas.
